All PVGRC members (regular and associate) are eligible for year-end rosettes. They will be awarded in 2026 for all AKC and GRCA titles awarded in the 2025 calendar year. Members are responsible for having their dog's titles in the correct order and with correct spelling of names and titles! You can look up your dog on the AKC website to get the proper listing order. Only AKC-issued or GRCA-issued titles or titles awarded through the AKC's Title Recognition Program (dock diving, barn hunt, therapy dog, etc.) can be included on your dog's rosette. Please remember to also double check the spelling of your dog's name.
This form allows you to submit up to eight titles earned in 2025 for each dog. If your dog earned more than eight titles in 2025, use the spaces for the next dog. In the space for the next dog's name, begin with the word "Continued" then re-enter the dog's name (without titles). Then enter the remaining titles for the dog in the additional spaces provided.
If you have more than four dogs, simply enter four of them on one form, then create a second form for the remaining dogs. Make a note of this in the Comments section at the bottom of each form.
After you submit this form, you will receive an automated response containing the information you have submitted. Please check it for accuracy, typing errors, etc. Also, Tricia Dunseith will email you back with acknowledgment that she has received your submission. If you do not receive an acknowledgment with 72 hours, please follow up with Tricia. If after you have submitted your form you realize that you have made a mistake regarding a dog you listed on the form (entered a title you earned in 2024, forgot a title, misspelled the dog's name, etc.), no problem! Just submit a new form and enter all general information (name, address, etc.) plus all the specific information on the dog for which a correction needs to be made. Do not re-enter any dogs that were submitted correctly on the first form you completed, just the dog that is being corrected. Then click the Yes button near the top of the form indicating that the form is a correction of the submission for . That's it!
The deadline for submission is 11:59 PM on January 12th, 2025. If a banquet is held in 2026, rosettes will be awarded then. If there is no banquet this year, your rosette(s) will be mailed to you.

Please note: You MUST arrange to pick up your rosettes at the banquet either in person, or by someone who you designate to pick them up for you. Rosettes will NOT be mailed to members who are not present or who have not made alternative arrangements to pick them up. You may use the Comments field at the bottom of this form to designate someone to pick them up, or to provide alternative means to get your rosettes. Any costs associated with alternatives must be borne by the club member.
